Sektion Becken- und Acetabulumsverletzungen

Aufgabenstellung

Im Juni 2024 wurde die ehemalige Arbeitsgemeinschaft Becken (AG Becken) nach mehr als drei Jahrzehnten erfolgreicher wissenschaftlicher Tätigkeit offiziell in die Sektion Becken- und Acetabulumverletzungen der Deutschen Gesellschaft für Unfallchirurgie (DGU) überführt. Dieser Schritt trägt der kontinuierlichen Weiterentwicklung, der wachsenden wissenschaftlichen Bedeutung sowie der langfristigen Ausrichtung der Gruppe Rechnung. Aus einer zunächst projektbezogenen Arbeitsgemeinschaft ist damit eine fest etablierte Struktur entstanden, die national und international als zentrale Plattform für die wissenschaftliche Bearbeitung von Becken- und Acetabulumverletzungen wahrgenommen wird.

Die Sektion verfolgt das Ziel, die Versorgung von Patientinnen und Patienten mit Beckenring- und Acetabulumfrakturen durch evidenzbasierte Forschung nachhaltig zu verbessern. Hierzu werden aktuelle klinische Fragestellungen identifiziert und in enger Zusammenarbeit zwischen zahlreichen Zentren multizentrisch bearbeitet. Neben Verletzungen nach Hochrasanztraumata rücken insbesondere osteoporotische Frakturen des älteren Patienten zunehmend in den Fokus der wissenschaftlichen Aktivitäten.

Ein zentrales Instrument der Sektionsarbeit ist das Beckenregister, das in Kooperation mit der AUC aufgebaut und kontinuierlich weiterentwickelt wird. Das Register ist eng an das TraumaRegister DGU® angebunden und ermöglicht eine standardisierte, multizentrische Datenerhebung über alle Versorgungsstufen hinweg. Aktuell umfasst das Register mehr als 11.000 dokumentierte Fälle aus über 40 teilnehmenden Kliniken, mit weiter steigender Tendenz. Ziel ist eine weitere nationale und internationale Ausweitung, um die Versorgungsrealität umfassend abzubilden und eine belastbare Grundlage für wissenschaftliche Analysen zu schaffen.

Auf Basis dieser Daten werden zahlreiche wissenschaftliche Projekte initiiert. Neben der Forschung stellt die Leitlinienarbeit einen zentralen Aufgabenbereich der Sektion dar. In Zusammenarbeit mit der Leitlinienkommission wird aktuell eine S3-Leitlinie zu Beckenverletzungen erarbeitet, mit dem Ziel, Diagnostik und Therapie auf Grundlage der bestverfügbaren Evidenz sowie der Registerdaten zu standardisieren. Diese Leitlinien sollen klare, praxisnahe Behandlungsempfehlungen und Entscheidungsalgorithmen bereitstellen und damit die Versorgungsqualität nachhaltig verbessern.

Die Sektion versteht sich als interdisziplinäre Plattform für den wissenschaftlichen Austausch, die Initiierung gemeinsamer Forschungsprojekte sowie die Weiterentwicklung der klinischen Versorgung. Durch die enge Verzahnung von Registerarbeit, Studienprojekten und Leitlinienentwicklung leistet sie einen wesentlichen Beitrag zur evidenzbasierten Behandlung von Becken- und Acetabulumverletzungen.

Im Rahmen der Sektion finden in etwa halbjährlichem Abstand eintägige Arbeitstagungen statt. Darüber hinaus kommen die Mitglieder einmal jährlich im Zuge des DKOU-Jahreskongresses in Berlin zu einer Sektionssitzung zusammen. Zwischen diesen Terminen werden Projekte in kleineren Arbeitsgruppen in der Regel per Videokonferenz weitergeführt.

Inhaltlich bieten die jeweiligen Tagungen einen Überblick über den aktuellen Dokumentationsstand der Datenbank. Dabei werden bestehende Dokumentationsprobleme sowie mögliche Änderungsvorschläge regelmäßig erörtert. Ein weiterer Schwerpunkt liegt auf dem Stand laufender Projekte sowie auf der Planung und Abstimmung zukünftiger Projekte und Studien.

Ergänzt werden die Tagungen durch vom jeweiligen Gastgeber organisierte Workshops, Fortbildungen sowie Fallbesprechungen zu besonders interessanten, gesammelten Fällen.
